Gardaí submit supplementary file with 'formal recommendations' to DDP on Creeslough

Gardaí investigating the fatal explosion at a building complex in Creeslough have submitted a supplementary file to the Office of the Director of Public Prosecutions.

The tragedy occurred on October 7, 2022, when an explosion at the local service station claimed the lives of five-year-old Shauna Flanagan-Garwe and her father, Robert Garwe (50), Catherine O’Donnell (39) and her 13-year-old son, James Monaghan. Leona Harper, aged 14, and Jessica Gallagher (24) were killed along with shop assistant Martina Martin (49), James O’Flaherty (48), Martin McGill (49) and Hugh Kelly (59).

"The supplementary file is in addition to the primary file submitted to the Office of the DPP in September 2025," An Garda Síochána said this evening. "The supplementary file includes formal recommendations from An Garda Síochána. The victims’ families continue to be supported by Family Liaison Officers.  Regular family liaison briefings are held, the most recent of which was this evening."

An Garda Síochána continues to appeal to any person with any information in relation to the fatal explosion on October 7 2022, and for whatever reason may not have spoken formally to An Garda Síochána to date , to please come forward and speak to the Garda Investigation Team now.

The Garda Investigation Team can be contacted at Milford Garda Station, where an Incident Room continues to investigate under the direction of a Garda Senior Investigating Officer at 074 9153060, the Garda Confidential Line 1800 666 111 or through Garda Station.

Families affected by the tragedy this week welcomed confirmation that they will have face-to-face talks with the Minister for Justice, Jim O’Callaghan, who has performed a u-turn and will meet in the coming weeks with family members and others affected by the explosion.
